Accessing Registration and Records Forms

Overview

Do you need to submit a course request, declare a minor, apply for graduation, or update your personal information? Below is a categorized list of the most used forms provided by the Office of Registration and Records. Each section includes a brief description to help you quickly find the form you need.

Registration and General Academic Forms

Form Name 

Purpose 

Prerequisite Waiver Registration Form 

Request to waive a course prerequisite with appropriate approval. 

Course Substitution Approval Form 

Allows substitution of a required course in your program under special circumstances. Department Chair or Dean approval required. Not valid for certificate coursework. 

Time Conflict Authorization Form 

Request to enroll in two classes with overlapping schedules. Requires faculty and department chair approval. 

Transfer Credit Approval Form 

Submit prior to taking outside courses for credit transfer. Must earn a ‘C’ or better and send official transcript to the Registrar. 

Cross Divisional Registration Approval Form 

Request to register for a course outside your academic division. 

Add/Drop/Audit Form 

Modify your course registration during the add/drop period or audit a course. 

Independent Study Request Form 

Submit a request to complete a course as an independent study. 

New and Readmitted Student Registration Form 

For students who are new or returning after a break. 

Degree Change and Academic Program Forms

Form Name 

Purpose 

Undergraduate Change in Degree Form 

Add, drop, or change your undergraduate degree. Advisor and Department Chair approval required. 

Master’s Change in Degree Form 

Modify your master’s program. Requires approval from Associate Dean or Department Chair. 

Doctoral Change in Degree Form 

Change doctoral program. Must be approved by the Dean or Director of Doctoral Programs. 

Minor Declaration Form 

Use to add a minor to your program. Follow all form instructions for processing. 

Graduation & Certificate Forms

Form Name 

Purpose 

Undergraduate Graduation Application 

Apply for graduation if you’re completing an undergraduate degree. 

Master’s Graduation Application 

Submit to begin the graduation process for a master’s degree. 

Doctoral Graduation Application 

Required for doctoral degree conferral. 

Certificate Request Form 

Request a certificate upon program completion. 

Replacement Diploma Order Form 

Order a replacement copy of your diploma. 

Graduation Information 

View commencement timelines, requirements, and procedures. 

Personal Records & Enrollment Verification

Form Name 

Purpose 

Personal Information Update Form 

Update your name, address, or contact information. 

Enrollment Verification Request Form 

Request proof of enrollment for employment, insurance, or loan deferment. 

Leave of Absence Form (Doctoral, MRes, MPhil) 

Submit a request for leave of absence from research-based graduate programs. 

Veterans Benefits Forms

Form Name 

Purpose 

Veterans Benefits at Capitol 

Overview of VA benefits available at the university. 

VA Certification Request – New & Readmitted Students 

Submit to receive VA education benefits after enrollment or re-enrollment. 

VA Certification Request – Continuing Students 

For continuing students to maintain VA benefits each semester. 

VA Schedule Adjustment Form 

Report changes to your schedule that may impact VA benefits.
