Tips for Submitting a Request

To ensure your request is handled quickly and accurately, follow these tips when completing each section of the form:

  • Title of the Work Request: Use a clear and concise title that summarizes the issue or request.
    • Example: Leaking ceiling in Room 210 or Request for event setup in Auditorium
  • How Can We Help?
    • Provide detailed information about the issue or service being requested. Include:
      • What the problem is (e.g., AC not working, light flickering, chair broken)
      • Where it’s located (building, floor, room)
      • Any relevant context (e.g., “started this morning,” “needed for event on Friday”)
  • Your Name, Phone Number, and Email
    • Fill out all contact information so the Facilities team can follow up if needed.
  • Where or What Is Having the Problem?
    • Use the search bar or dropdown menu to select the affected location or asset:
      • For rooms or offices, enter the room number (e.g., 203B)
      • For shared spaces, try keywords like “restroom,” “hallway,” or “lobby”
      • If you can’t locate your exact area, choose “I’m not sure” and type the specific location into the Title or How Can We Help? field
  • Requested Completion Date: This field auto-fills with today’s date. If you are submitting a request for a future event (e.g., setup needs), you may update this to reflect your preferred completion date.
  • Attach File: Upload a photo that helps clarify the request. This is highly encouraged for visual issues (e.g., broken items, leaks, setups).

QR Code Submissions

QR codes will be posted around campus in high-traffic areas like restrooms, hallways, classrooms, and offices. Utilize them to submit a work request.

Common Request Types

Some common examples of requests include:

  • HVAC Issue
  • Lighting Problem
  • Leak or Water Damage
  • Door or Lock Malfunction
  • Furniture Repair or Assembly
  • Restroom or Common Area Issue
  • Event Setup Request
  • Other (please specify)
